Welcome to Henrico Family Physicians. Our practice is dedicated to providing comprehensive and compassionate medical care for you and your family. We strive to do this through:
1. Preventive care and advice to keep disease from occurring.
2. Acute care to treat sudden illness or injury.
3. The treatment of chronic medical conditions to lessen severity and prolong quality life.


Our office motto is “Your Medical Home.” We want to develop a place where you can come for information and advice on things medical. We combine today’s technology with old fashioned caring to accomplish this goal.


Family Medicine is a medical specialty that centers on you and your family. As board certified Family Medicine Specialists, we have completed advanced training to provide you and your family with the majority of care that you may need. We provide a complete spectrum of care in our office including:

..... Physicals for school, sports, work, camps, and insurance.

..... Chronic disease management including high blood pressure, diabetes, and elevated cholesterol.

..... On site EKG and laboratory testing.

APPOINTMENTS AND SICK CALLS

Our office is open from 8:00 AM to 5:00 PM M / W / F and 8:30 to 5:00 on Tues / Thurs. Other times may be available by prior arrangement.  If you are sick and need to be seen immediately, let us know as we will make every effort to see you the same day. If you need an appointment for chronic care, we will be happy to schedule an appointment during our normal hours at your convenience.  We also ask that new patients arrive 15 minutes prior to your scheduled appointment time to fill out necessary paperwork.  If you cannot keep a scheduled appointment or will arrive late for an appointment, please call the office.


AFTER HOURS

Our office has a physician on call 24 hours a day for emergency conditions. If you require hospitalization for any reason, we will work closely with the in-hospital specialists and coordinate your care after your discharge. We frequently use either Henrico Doctor’s or St. Mary’s as our primary hospitals, but we will be happy to coordinate with your hospital specialists at any local hospital that you may choose.


INSURANCE AND PERSONAL INFORMATION

Please be familiar with the type of insurance that you currently have. We must have current insurance and personal information (including a picture ID) to correctly file your insurance. If our information is not correct, you risk non-coverage for care by your insurance company. We accept most major insurances, please confirm that we accept yours. 


MEDICAL FORMS

If you have medical forms of any kind that need to be filled out by your doctor, let the receptionist and your doctor know. Forms will normally be completed in 72 hrs. There may be a charge for some forms, please ask the receptionist or the office manager for details.


PRESCRIPTION REFILLS

All requests for prescription refills must be called in during regular office hours. Refills will normally be completed within 24 hours so please allow appropriate time. Prescription refills and referrals will not be completed after hours.

 

REFERRALS

Occasionally, your condition may warrant advice or treatment from another medical specialist.  As per most insurance guidelines, you must be seen and evaluated by our office prior to obtaining a referral to a specialist.  Referrals will not be completed without an office visit except in medical emergencies.  Most insurance companies will not allow referral forms to be completed after you have seen the specialist except in rare emergencies or special situations.


LOCATION

Henrico Family Physicians is located in the new Mayland Medical Complex at 3460 Mayland Court, Suite 100.  Mayland Court is approximately 1 block east of Gaskins Road on Mayland Drive.

Dr. Schwartz and our staff welcome you to Henrico Family Physicians. Born and raised here in Henrico, Dr. Schwartz earned his medical degree from the Medical College of Virginia in 1992. After completing specialty training in 1995 at the Blackstone Family Practice Center in Blackstone, Virginia, he moved back to the Richmond area and has provided family medicine and urgent care ever since. Dr. Schwartz is board certified in Family Medicine by the American Board of Family Medicine. He is also a Flight Surgeon in the Virginia Air National Guard. He and his wife Angela have two sons, David and Cooper.
